STARS Parents,
Please click on the link below to fill out your STARS Medical Registration Form for 2010-2011. This form is due September 13, 2010 so that we have accurate information for the fall.
Also, this form has a time limit of 30 minutes for your protection, so make sure to have all medical information in front of you before you start.
